Requirements of Withdrawing Certificate and Academic Transcript

1. After Publishing the result of all required credit hours.

2. Issuance of Academic Transcript is necessary before or along with certificate.

3. Show original and submit photocopies of previous academic certificates.

4. ID card and library card must be surrendered during DIA (Document Issue Application) Form Clearance.

5. Must have accounts clearance.
